List media library items

The unified library: clips, highlights and streams under one shape, with
the same filters the Zentag Studio library uses.

Filters combine with AND across parameters and OR within one, so
players=p1,p2&rating=5 means "either player, rated 5". All list-valued
parameters are comma-separated.

Pair this with /media/filters/counts to build a filter UI: that
endpoint takes the same parameters and reports which values remain
selectable, and /media/stats reports the totals for the selection.

Paginate with cursor, not page. The cursor is opaque - pass back
next_cursor verbatim and do not parse it.

Result ordering. A cursor is tied to the ordering it was issued under, so changing this mid-pagination invalidates it.
